After scheduling a complimentary consultation appointment our patients receive a welcome email containing a link directing them to fill out 'new patient' forms. Clicking the link prompts OrthoFi to open and asks the patient to create a password and fill out a series of forms. We ask that all our new patients fill these forms out prior to arriving for their consultation in an effort to minimize the time spent on paperwork during the appointment.
